Patients under 18 years of age should be accompanied by a parent or legal guardian, otherwise you will need to complete the Consent to Treat a Minor form

Your Medications
If possible, please bring all of your medications, vitamins and supplements in their original container(s) to your first appointment.
Some medications, such as those for chronic pain or anxiety, may not be filled during your first visit in accordance with our controlled-substance policy.
Once you've established care with us, you'll request refills directly from your pharmacy. To make sure you don't run low on your medications, please plan for a 48- to 72-hour processing time.

Need your provider to complete a form?
If you need your provider to complete a form (return to work form, approval to join a gym, etc.), you can upload the form to us using the My Navigator patient portal.
