EvergreenHealth Monroe is excited to offer medical and prescription benefits through Premera Blue Cross. The plan offers a wide range of in-network providers and enhanced benefit coverage if you see a provider with in the EvergreenHealth Expanded network. We pay 100% of the employee’s medical insurance premium and 50% of dependent premiums.
EvergreenHealth Monroe provides vision insurance through VSP. VSP hase a large network of providers and our plan has minimal out of pocket cost. Our plan includes an annual well vision exam and your choice of frames or contacts every year, with low out of pocket cost.
EvergreenHealth Monroe provides a choice between Premera or Willamette dental insurance plans. Each plan provides orthodontic coverage. There is no charge to employees for dental coverage for themselves and the hospital pays 50% of the dependent premium costs.
EvergreenHealth Monroe’s Employee Assistance Program (EAP) provides short-term support designed to assist employees in handling personal and work related issues. Free confidential counseling support is provided in areas such as anxiety and depression, child development, divorce, emotional concerns, relationships, terminal illness and work-related problems.
EvergreenHealth Monroe provides employer-paid basic life insurance coverage which includes a cash benefit to the employee’s beneficiary in case of death. Employees have the option to increase coverage on their policy or to purchase coverage for their dependents.
EvergreenHealth Monroe offers a long-term disability (LTD) insurance plan to provide supplemental income if an employee is unable to work. Premiums are 100% employer-paid for this insurance.
EvergreenHealth Monroe offers both a 457 Tax Deferred Compensation plan and a 401(a) Retirement Plan options to help employees save for their future. We provide a matching contribution to an employee’s 401(a) plan at $2 for every $1 contributed (up to plan maximums).
Employees can set aside money on a pre-tax basis to pay for qualified dependent care and/or health care expenses through a Flexible Spending Account arrangement. Pledge amounts are made by employees on an annual basis.
Employees are able to take paid time off for vacations, holidays and illness/disability through the Paid Time Off (PTO), Sick and Extended Illness Benefit (EIB) benefits. The amount of time off an employee accrues is based on job classification and work status.
